The excludes are relative to the tree root.
So not "-/var/www/sites/*/*/Mailboxs" but "- /sites/*/*/Mailboxs"
(note also the required space after the "-" !)

And do you really have a var/cache under your /var/www/ ?
It seems to me that this is a copy from a root config, so you can remove
the first 4 lines as well.

Additionally, the way the rules look now, I think the last + line isn't
needed as per default things are included anyway.
